Pain on outside of ankle

Hello doc! I'd like to ask about some ankle pain I have. I developed it after I ran a Marathon last year. It comes and goes, and I haven't figured out how to keep it away.

Its pain on the outside of my left ankle, sometimes behind my ankle, sometimes in front. It get worse with activity. It hurts to bear weight on it, and hurts to pronate my foot inward.

I have flat feet and my feet pronate inwards when standing. I also have big toes bent inwards. I run landing midfoot. I didn't have any ankle problems the 5 months prior to the Marathon, during my training. It started when I hit the 30K in my training schedule.

After the Marathon I wanted to keep up half marathon runs every weekend, I loved those. It felt like the sweet spot. But now with this pain I do 5k once a month at best. Its very sad :(

I'm doing ankle strengthening exercises in the form of balance exercises. What else can I do? Any idea whats wrong? And what is a likely cause?
 
Hi Lital,

Because the pain is moving around, it is difficulty to give you a good answer. There is likely a problem at the joint between the fibula and talus, or between the talus and calcaneus. Since this pain has been around so long, and is severely limiting your mobility, my best advice is to have it professionally evaluated in person, likely with some xrays.

It is likely that the problem may not be in the ankle at all. This can be a core issue (are your hips distorted?). Pronation is falling in and turning outward, supination is turning inward so regarding your flat feet, it is likely a core problem is causing you to slam your feet into the ground, especially on the side of pain.

Chasing the pain is a lesson in futility and is likely to give you or any practitioner helpful info on resolving this. If you would like to post a video of you walking from the side and from the back, I can give you some useful information based on what I see. A treadmill video can be quite helpful.

Andrew - I had X-rays last year. They found nothing. I found it funny when they wrote "diagnosis, flat foot" on my paperwork.

William - That is a brilliant idea, thanks for that offer. I don't have access to a treadmill, but I will get upload videos from the front and back, I hope today.
