I suspect a lot of doctors go with the "evidence-based medicine" that was around when they graduated from med school. They kind of try to keep up, but are too busy seeing patients.

And then there is a lot of folklore (yes, I am pointing at a lot of podiatrists) that just gets passed down from doctor to doctor pretending to be evidence-based medicine. What they do was just never based on studies.
